SSN-RC-2021-006 - Youth and Family Services in Rockland County- Nyack Center

The County of Rockland is considering the issuance of a Sole Source award to Nyack Center for the provision of Youth and Family Services to residents of Rockland County. Funding amount is contingent on the adoption of the County of Rockland 2021 Annual Budget. As such, final funding amount may differ from the estimated amount.

TERM: January 1, 2021- December 31, 2021

This award may be made on a sole source basis in accordance with the Rockland County Procurement Policy.

The provider shall perform the following Program/Services:

• Serve at least 150* new, unduplicated children and teens by furnishing daily homework assistance, computer access, mentoring, and literacy services.
• Operate a breakfast club- children will be able to start their school day with a healthy breakfast
• Advocate for people in need and develop responsive programming in the community
• Collect data through Breakfast Club Attendance Sheets
• 80% attendance rate outcome in the breakfast program
• Services provided by French-Creole and Spanish speaking staff
• Provide services Monday – Friday, 7:00 a.m. – 8:30 p.m.
• Data will be collected through After School Program homework logs
• Track homework in after school program for a 75% completion rate
• Homework support for Rockland County youth in grades K – 8.
• Computer skills training for youth.
• Mentoring services for youth.
• Support services for low-income and underserved youth and their families including:
  • Education
  • Immigration
  • Translation
  • Civic engagement
• Provider will refer clients, as appropriate, to the New York State's MyBenefits website, to inform them about various assistance programs and supportive services 
• Site for workshops and trainings through the Nyack Public Schools.
• Provide services that assist low-income families to achieve self-sufficiency.

• Follow state regulations for licensed school-age-care
• Provide priority to low income families
• Verify low income by obtaining tax forms, paystubs and proof of address to register in the program
• Demonstrated successful experience providing the above services to eligible

• At least 90% of families served must be at or below the Federal Income Guidelines for poverty.
• Divert 100* families from applying for or continuing to receive Department of Social Services Temporary Assistance benefits.
• Provide Information and Referral Services for 1701* unduplicated families.

• • Provider ensure that its information, and all updates, are provided to and contained on the “2-1-1- database system”. Provider will contact Hudson211 to complete the required forms within thirty (30) days of contract execution; [2-1-1 is a program of United Way in the Hudson Valley].

• Provider must include a written attestation that it will be delivering one or more of the following services to all of its clients, as applicable, under the resulting contract:

• Child Preventive Services (referral, monitoring supportive and rehabilitative services provided to children under the age of 18 and their families to avert disruption of family unit)
• Child Welfare Services (services to protect and promote the welfare and safety of all children, including individuals with disabilities; homeless, dependent or neglected children)
• Family Preservation Services (services for children and families designed to prevent harm to children and help families at risk or in crisis)
• Family Support Services (community-based services to promote the well-being of children and families designed to increase the strength and stability of families and enhance child development)

• be located in Rockland County and serve all Rockland county communities
• Experienced in providing the same or equivalent services
• have Spanish speaking and French-Creole speaking staff for this program
• be accessible by public transportation
• Must be site for WIC program through the Rockland County Department of Health.

• Be governed by a Board of Directors
• Be up-to-date with all IRS filings
• Be up-to-date with all NYS filings
• Have a 2021 budget with Board approval
• Be fiscally sound
• Have a Non-discrimination Policy
• Have a Conflict of Interest Policy
• *Provide a copy of agency’s most recently filed 990 IRS Form

• Maintain program records for the purpose of an audit and provide to the County all program-related information including proprietary materials, upon request
• Submit quarterly and year end reports, including attendance lists for each group activity, and detailed log sheets for all clients served
• Submit quarterly vouchers to RCDSS
• Quarterly Vouchers, which may only include expenses for the respective quarter, that are not supported by the proper, accurate documentation and/or not provided to RCDSS within thirty (30) days of the end of each quarter, or upon request by RCDSS, will not be considered for payment by RCDSS.
• Be available for on-site visits, announced and unannounced

• Demonstrated successful experience providing the above services to eligible individuals and families by records indicating Performance Target Outcomes have been met
