Description

The purpose of this RFP is to obtain proposals to strengthen the re-entry planning and transition process for incarcerated individuals from the Ulster County Jail back into the community. While there is currently a full-time jail discharge planner through PrimeCare Medical, Inc., they are unable to support people after they leave the jail facility. The proposed jail re-entry case manager will work in collaboration with PrimeCare and other partners assisting with the jail discharge and re-entry process throughout the county.
