Description

Proposals are requested for a comprehensive dental insurance program to provide PPO and/or DHMO Dental coverage. The proposal may be submitted for one or both of these types of plans.
Proposals are sought for dental coverage from January 1, 2025, through August 31, 2025, and then for each fiscal year thereafter beginning September 1, 2025. The rating period will be on a fiscal year basis. The plan year will be on a calendar year basis.
