Sealed proposals will be received on or prior to 11:00 a.m. on May 12, 2022, at the Business Office, East Islip Union Free School District, 1 Craig B. Gariepy Avenue, Islip Terrace, New York. Proposals will be opened and evaluated in private. Proposals received after stated date and time will be returned to the sender, unopened.
All proposals must be submitted on or before the date and time of opening in sealed envelopes, bearing on the outside the name and address of the proposer and the title of the RFP: RFP #051222-2 School Physician for Athletic Healthcare Services
Specifications and proposal forms may be obtained from the district’s website www.eischools.org/district/bids, Empire State Purchasing Group’s website www.bidnetdirect.com, or at the above office, weekdays, between 8:30 a.m. and 3:00 p.m.
Proposals shall be irrevocable for a minimum period of forty-five (45) days from the date of proposal opening. The Board of Education reserves the right to reject any and all proposals, or to award contracts which, in its judgment, are best for the school district.
Requests for Information must be submitted in writing and will be received until Thursday, April 28th, 2022 at 12:00 p.m. These requests may be emailed to the Purchasing Agent, Samantha Neff, at Samantha.neff@eischools.org.
