Summary of Specifications:
The following is the criteria for the replacement of the inclined stair wheelchair lift at Ralph R. Smith Elementary School located on 30 Smith Court Hyde Park, NY Work is to be completed by September 1, 2015. Preliminary measurement, planning and ordering should be done so that offsite fabrication can be completed prior to June 29, 2015. Scope of work includes the following: • Dismantle and remove the existing unit. o District to identify and retain working parts to be used elsewhere. o Seal, patch and blend any blemishes left from the original installation. • Supply and install a new three stop, two charging/control station unit that is ADA compliant. Unit to travel over two flights of switchback stairs covering three levels with a minimum capacity of 550 lbs carrying load. • Provide all electrical and safety control work needed to operate the unit. Supply and install localized service disconnect. • Installation work is to be verified and approved by manufacturer representative with District representatives present at time of completion. • District will assume ownership of the unit upon inspection by independent certified inspector. Cost of inspection to be covered by the District. • Provide training on new unit to HPCSD building staff. Persons to attend to be determined by the District. Provide two copies of the Operation and Maintenance Manuals to include all shop drawings, electrical schematics, parts list, manufacturer warranty, and as built drawings. • Vendor to supply all labor at NYS DOL prevailing rate and provide certified payroll with payment submittals. • There will three payments for this project. o The first will be upon completion of the rail fabrication which will cover 35% of the total cost. o The second will be upon completion of the installation which will cover 55% of the total cost of the project. o The third and final payment of 10% will be made after all training, inspections and documentation have been turned over to the District as described herein. • Contractor to sign "Hold Harmless Agreement" supplied by the District. • Contractor to supply proof of insurance with the District listed as additionally insured, per the terms listed in attached packet. • Contractor to supply one year of warranty on the labor portion of the project. • Contractor to supply two years of service and maintenance for the unit from the date it is turned over to the District. o Service agreement to include no more than two hour response time for emergency service and repair calls. o Regular maintenance to be performed per manufacturer’s specifications outside of regular school day hours.
