Description

The Rensselaer County Unified Family Services – Department of Aging is interested in contracting with a Home Health Agency Care Firm (provider) for the services listed in this Request for Proposal (RFP). The provider agrees to provide Personal Care I and II and Ancillary Services in accordance with the attached specifications to Program Clients. The provider will comply with the requirements contained in 766.3 (Personnel Requirements) of the New York State Code of Rules and Regulations and written evidence of the compliance is available to the Department and the State Health Department.
