RFP-TFS-032522 EAP - RCSD Employee Assistance Plan - 2O22

With this RFI the Rochester City School District (RCSD) is requesting information regarding your company and your services. The same information will be gathered from different companies and may be used to determine with whom the RCSD will contract for services.
 
The intent of the contract is for a minimum of a five-year period. The contract will be a one-year contract with options to renew for (4) four additional one-year periods.
 Requirements
The proposed Employee Assistance Program response should include at a minimum

• A description of how these requirements will be met by your team.
• Complete Attachment A (Requirements Matrix) – 3 tabs,
• Provide any additional services that you feel would benefit the District

1. Unlimited telephonic counseling, 24/7
2. Up to 6 face-to-face counseling sessions per issue, with access to local or national staff counselors if necessary
3. Work/Life benefits including debt, legal, financial, elder and child care counseling
4. Lifestyle benefits designed to improve member’s health, financial security and overall well-being
5. Unlimited Administrative (mandatory) Referrals
6. Statistical reports, newsletters and EAP Awareness Materials
7. Provide onsite assistance/trauma response in the event of a catastrophic event or employee/student death
8. All web services for employee and employer
9. Provide onsite assistance during employee informational/wellness events.
