RFP-DF-27-2012 - Community Clinical Alliance

Summary of Specifications:
The Schenectady County Public Health Services’ Clinic is licensed by the NYS Dept. of Health as an Article 28 Diagnostic and Treatment facility to provide limited diagnostic and treatment services in Schenectady County. SCPHS is interested in forming a Community Clinical Alliance with one established Article 28 Schenectady County health care provider which will provide certain of its’ public health clinic services to Schenectady County residents. Service site must be centrally located and accessible by public transportation for clients. The primary guarantee from facilities will be to assure access to selected services to un- and under-insured clients regardless of patient’s ability to pay (described below). Schenectady County Public Health Services (SCPHS) will actively monitor and evaluate the Community Clinical Alliance’s services provided to residents of Schenectady County as part of its public health mandate.
