Quotations are requested for furnishing the items described below in accordance with terms set forth herein:
Proposal A – WEED CONTROL – Downtown Village Center
The Village is desirous of eliminating all weeds on identified cobblestones, brick pathways and other planting areas in the Downtown Village Center. See attached map for all locations. On identified cobblestones, brick pathways and other planting areas, a pre-emergent treatment of Surflan or equivalent should be applied in April and a post-emergent treatment of Razor or equivalent should be applied in July. Exact timing of application must be coordinated with the Highway Foreman or his designee.

The Village is desirous of eliminating poison ivy from locations throughout the Village. These locations have been annually treated for poison ivy and small amounts emerge each year. See attached list of 28 locations. The lump sum price shall be only for one spraying to be coordinated with the Highway Foreman or his designee. The hourly rate is if additional spraying of poison if required at a later time.

ADDITIONAL REQUIREMENTS:
The Village May Include Additional locations at its discretion and in conjunction with and by the approval of the contractor.
Contractor must posses the necessary pesticide application license (3A, 6A) and comply with all Federal, State, County, and Local Regulations.
Insurance Requirements:
Certificates of Insurance (Contractor Insured) a) Workmen’s' Compensation (Sec. 57 W.C.L. Form C 105.2) b) Disability Benefits (Sec. 220(B) D.B.L. Form DB 120.1) c) General Liability I) Bodily Injury - 3,000,000 II) Property Damage - 1,000,000 A certificate of insurance naming the Village as additional insured, in form satisfactory to the Village Attorney, with limits for personal injury of not less than $1,000,000 for any one person and not less than $3,000,000 for any one accident and not less than $100,000 for any one accident and $1,000,000 in the aggregate for property damage, insuring the village, its officers, employees and agents against any liability for personal injury or property damage, directly or indirectly resulting from or arising out of the granting of any such permit or any such opening or the method and manner of doing any work permitted or required by any such permit or under this Article or any negligent act or omission in connection therewith on the part of the permittee, his employees or agents, provided that a public service corporation may, at its option, file an undertaking, in form satisfactory to the Village Attorney, to indemnify and save harmless the village, its officers, employees and agents from any such liability, covering all openings made by it.
