Summary of Specifications:
INTENT: It is the intent of this proposal to secure a bid for Dairy Products per the attached specifications for various Warren and Hamilton County sites from November 1, 2015 through January 31, 2016 with the possibility of extending the exact bid terms for an additional three month term with the approval of both Vendor and County.
SCOPE: This contract shall cover Dairy Products for both Warren County and Hamilton County sites. These include Countryside Adult Home, 353 Schroon River Road, Warrensburg, NY 12885, Warren County Sheriff’s Office, 1400 State Route 9, Lake George, NY 12845, Westmount Health Facility, 42 Gurney Lane, Queensbury, NY 12804 and all Eight Office for the Aging Meal Sites. Please see attached sheet for location of meal sites. Any other designated County facility that may be added during the term of this bid will be added to the vendor serving the closest nearby site.
Please note that Warren County is currently in the process of selling Westmount Health Facility. If the facility is sold prior to the end of this bid, the County reserves the right to terminate the bid for said facility.
