Bidder must provide proof or evidence of the following with the submittal of their quote for services:

1. Bidder must have in-house engineering personnel that possesses a Professional Engineer (PE) License in the State of NC—include PE License Number (#) that will oversee this project.

2. Bidder must have in-house concrete NACE-certified level 1 and 2 inspector, who also possess a current OSHA 30 Certification on staff. This person will be responsible for monitoring the project from start to finish and providing City Staff with written weekly updates on the project.

3. Painting crew must demonstrate a minimum 15 years of combined experience in the state of North Carolina, in concrete water tank remediation.

4. Bidder must provide a description of the proposing company’s Safety Program, PPE Hazard Assessment & waste program prior to conducting any work.

5. Bidder must submit a copy of their ISO 9001:2015 Certification.
