The Warwick Police Department seeks to solicit Bids from licensed medical facilities for the pre-employment medical exams for our new recruits.

REQUIREMENTS:

The winning bidder shall supply the following service/exams:

Pre-employment Physical (PEP)
9-Panel Urine Drug Screen (DSC)
Tuberculosis Test (PPD)
Completion of the supplied State of Rhode Island Examination Report (See attached)
Blood Type Testing
